REQUEST FOR ePASSPORT VALIDITY EXTENSION (Per CIR-516-OUA-2012 dated 27 March 2012 and CIR-168-OUCSCC-2016 dated 5 January 2016

Filipinocitizens living within the jurisdiction of the Embassy or travelling in Jordan may come to the Consular Section to:   

  •    renew an expired/expiring Philippine passport; or  
  •    replace a lost/damaged Philippine passport

Important: The Embassy has completely switched over to the new ePassport and no longer processes the obsolete green passport (MRRP) nor the maroon-covered machine-readable passport (MRP). However, recently issued MRPs would still be valid until the date of expiry. The ePassport is the current international standard img1as prescribed by the International Civil Aviation Organization (ICAO). Read the advisory on ePassport implementation.

Apply for renewal or replacement of your current passport at least one (1) month before you need it since turnaround time for the ePassport is

between thirty (30) to forty-five (45) days. Personalization and printing of the ePassport is centralized in Manila. No need to bring photos unless you wish to extend the validity of your current passport (in which case bring two photos with you).

It is the responsibility of the applicant to apply early enough to allow for typical processing times so that the planned travel would not be delayed.

     FACTS ABOUT DFA'S PASSPORT SERVICES

 

1. What is the DFA appointment system?

It is a system developed by the DFA that allows the passport applicant to schedule a specific date and time to have the ePassport processed.

2. Who may avail of the appointment system?

All eligible Filipinos who wish to apply for an ePassport at the DFA Consular Office in Aseana Business Park (corner Bradco Street and Macapagal Avenue, Parañaque) near Mall of Asia and Baclaran Church.

 

  1. Passport Application Form, duly accomplished and signed by applicant, or if applicant is below eight (8) years old, signed by a Filipino parent or the legal guardian (who must present proof of guardianship), with the minor applicant affixing left and right thumb prints;
  2. Police report with official English translation from a licensed translation office must be submitted with passport application form, but only if lost passport is still valid;
  3. Affidavit of Loss or Affidavit re Damaged Passport stating personal and passport details, as well as the circumstances on how passport was lost or damaged, executed by applicant and duly notarized by a Consular Officer;
  4. One (1) photocopy of the passport data page, if available, or the NSO-issued birth certificate (on security paper), should also be submitted to speed up the verification and clearing process;
  5. Three (3) identical 3.5cm x 4.5cm passport photos that comply with the following:
    • taken with white background;
    • taken within the last six (6) months;
    • showing full front view of the applicant's face;
    • applicant in decent attire (with collar and sleeves);
    • preferably without glasses (or if with glasses, must have no reflections);
    • male applicant should have no earrings
  6. Personal appearance of applicant is required except:
    • if applicant is below eight (8) years old, in which case application must be filed in person by a parent;
    • if applicant is over sixty-five (65) years old, in which case application must be filed in person by an authorized next of kin; or
    • if applicant is incapacitated (mentally or physically), in which case application must be filed in person by an authorized representative who must show medical certification regarding applicant's condition
  7. Authorization letter signed by passport applicant and designating representative is required in the three exceptions cited in preceding paragraph.
 

Important: Replacement of lost passport is subject to a 15-day clearing period during which prior issuance of the passport reported as lost is verified.

 

Additional Requirements for Passport Applications

The following additional supporting documents are required for Filipino seafarers who wish to renew an expired or expiring passport or replace a lost or damaged passport:

  • Present original of Seaman's Book and submit one (1) photocopy of the data page; and
  • Letter/Certification from the Ship Master or Corresponding Shipping Agency in Jordan attesting that passport applicant is in fact a crew member; with a
  • Copy of the Crew List, indicating therein the name of the passport applicant
